Board of Health Flu Clinics Flu shots are available free of charge to anyone age 3 and older. Children under age 18 must be accompanied by parent or guardian. There are no residency restrictions. Those age 65 years and older must provide a health insurance card from Medicare or a participating Medicare HMO Plan. Please bring your Medicare card and your Harvard Pilgrim, Tufts Medicare Preferred, Fallon Senior Plan, BC/BS Medicare HMO Blue, BC/BS Medicare PPO Blue or Senior Whole Health card (if covered by one of those plans.) Your health insurance number is being requested for reimbursement purposes. You will be required to show your card before receiving your vaccine. You may pick up a Medicare Reimbursement form at the following locations: Senior Center, Recreation Department and Board of Health office – all at 61 Center St., Town Hall lobby and Burlington Public Library.
