How may we help you?

Birth, Death or Marriage Records

 Certified copies of the following vital records are available from the Town Clerk's Office for a fee of $10.00 (7/1/01). Requests for copies may be made in person at the office, through the mail (click on the record type listed below for a mail in application) or Online

Births - persons whose parents lived in Burlington at the time of their birth
or whose birth occurred in Burlington;
Deaths - persons who died in Burlington or who died in Massachusetts
and listed Burlington as their place of residence;
Marriages - persons who filed their Intentions of Marriage in Burlington.

 
NEW - You can now request and pay for most Burlington Vital Records Online.

The Online Vitals Request system is available thru MCC eBill and allows you to make requests and pay for them using a credit card or electronic check.
This system does require one time setup and Log-in that you create.

**NOTE**

1) Online Requests are:
$15 per record for regular processing - One week turn around,
$30 per record for Rush processing - 48 Hour turn around.
Also, there is a non-reimbursable convenience fee per record
of up to $3 on Credit Cards and 25 cents for E Check payments

2) Some records are restricted (impounded), i.e. parents were not married at the time of the birth or the father is not named on the record. Only those listed on restricted records are entitled to obtain a certified copy and identification is required. Please call the office at 781-270-1660 prior to ordering such records online. You will be asked to fax a copy of a valid photo ID along with your signature.

3) If we do not have the requested document in our records the processing fee will be reimbursed by the Town via a check. This will take up to 4 weeks.

   Marriage Licenses  

Persons wishing to obtain a license must file their intentions with the Town Clerk in person (except for in cases where one person is in the military or incarcerated) at least three days before the ceremony.  Court waivers to the three-day waiting period may be obtained at the Woburn District Court or the Probate Court in Cambridge
Couples obtain a letter from the Clerk to bring to the Court. The cost of a license in Burlington is $25.00, which includes a certified copy. The license is valid for sixty days, and may be used for a ceremony conducted in any Massachusetts city or town. The member of the clergy or Justice of the Peace who performs the ceremony must sign the license and return it to the city or town where it was issued. The completed license is then sent to the State Registry of Vital Statistics.  Certified copies of the record must be obtained from the city/town where the intentions were filed or from the
State Registry of Vital Statistics.

Burial Permits

Burial Permits are issued from the Town Clerk's Office during regular business hours - 8:30 AM to 4:30 PM.  If possible, fax the death certificate in advance to 781-238-4692.

When the Town Clerk's Office is closed, permits are available at the
Police Station, 45 Center Street. 
Do not leave the death certificate at the Police Station -
deposit certificate in the mail box on the steps by the front door of Town Hall. 
Permit fee - $10.

Business Certificates

All business certificates (commonly known as "doing business as" or "DBA" certificates) are filed with the Town Clerk. The DBA files allow consumers to identify and locate the proprietor of a business that goes by a name other than that of the proprietor. Once filed, a certificate is valid for four years. The fee for filing is $40.00(7/1/01).
The Town Clerk offers a subscription service for $100/year. Subscribers receive a weekly listing of new businesses. Send your check made payable to the “Town of Burlington” to the Town Clerk, 29 Center St. Burlington, MA 01803.

  Dog Licenses

Dog License Application

All dog owners in the Town must obtain a license for their dog by the May 31st each year. A license will be issued only if the owner has evidence of a rabies vaccination that is valid as of the date of issuance. The fee for a dog license is $15 for 1 year, $20 for 2 years, $25 for 3 years. The Town has a leash law and pooper scooper law.

Rabies Clinic sponsored each March/April by the Board of Health for all cats and dogs.

  Zoning & General Bylaws/Maps  

The fee for copies of the Town Bylaws is $8.00 and $2.00 each for zoning/aquifer/wetlands and telecommunications overlay maps. The Bylaws are also available on this website for no charge (see below).  Send your check made payable to the “Town of Burlington” to the Town Clerk, 29 Center St. Burlington, MA 01803.
